In today’s world of fast paced communication and information overload, getting the right information to
the right person in a timely, effective and comprehensive manner can be daunting. It is however, not
impossible. In this module you will learn basics of how and what to communicate to team members and
other health care providers.

- Evaluate potential communication road blocks and propose strategies to avoid them.

Review Leonard Patrick
Your patient, Leonard Jones just completed a 3 months cardiac rehab program at your facility. He is
55 years old, has had a myocardial infarction and underwent PCI. His lipid levels and anthropometric
measures are higher than normal. He is not adhering to his exercise goals and he missed 40% of the
program due to work issues. His Hospital Anxiety Depression Scale (Depression Test)
indicates mild depression.

list the key components that his primary care physician would want you to include in a discharge summary.

- Have a colleague evaluate the discharge summary you have developed.
